Community Health Data Platform

As one of the nation’s largest nonprofit integrated health systems, Kaiser Permanente is uniquely positioned to advance the health and well-being of the people and communities we serve. Every 3 years Kaiser Permanente conducts community health needs assessments (CHNAs) to guide and inform investments that improve health equity. The CHNAs are a critical tool for identifying and prioritizing community needs and assets in the eight regions Kaiser Permanente serves.

To support this work, CCHE developed the Community Health data platform, which includes 85 measures organized into a set of health topics based on the County Health Rankings population health framework. The measures include census sociodemographic data, results from national health surveys, and a variety of other data showing social, environmental, and health factors and outcomes. Around half of the measures are available at the ZIP code level for each CHNA service area, allowing users to explore geographic and racial/ethnic disparities within each community.

Key structural decisions for the platform include having a data framework that facilitates prioritizing community needs, using publicly available quantitative data from national sources, and providing a user-friendly way to explore the data. We chose Tableau because it is easy to use and highly interactive — tables and maps can be filtered by both health topic and geographic area. The initial release of the 2024 version of the data platform is organized by Kaiser Permanente region and service area. Future releases will include data for all counties in the states Kaiser Permanente serves.
